I looked into this issue a bit more.

It seems as though it could be compiled with openjade, for example
http://tldp.org/HOWTO/DocBook-OpenJade-SGML-XML-HOWTO/x456.html, but
during compilation to HTML (before going to PDF), I get the following
errors

 reference.xml:8:2:E: cannot find "../local.ent"; tried
"../local.ent", "/usr/local/share/sgml/../local.ent",
"/usr/share/sgml/../local.ent"

penjade:reference.xml:120:0:E: cannot find
"../shared/hosts_pools_intro.ent"; tried
"../shared/hosts_pools_intro.ent",
"/usr/local/share/sgml/../shared/hosts_pools_intro.ent",
"/usr/share/sgml/../shared/hosts_pools_intro.ent"

Plus lots of other undefined references.

The command I tried was:
openjade -t xml -d
/usr/share/sgml/docbook/stylesheet/dsssl/ldp/ldp.dsl#html
/usr/share/xml/declaration/xml.dcl reference.xml

Any ideas on how we can get this working?
