[IA64] optionally enable xen console with irq/input.

Makes xen serial console working in input.
You need to add 'xencons' option to xen command line, and you need
to disable serial on linux (unfortunatly).
Remove unused functions in irq.c
Add a ia64_xen_vector bitmask of vectors used exclusively by Xen.
