[xen master] VT-d: qinval indexes are only up to 19 bits wide

    VT-d: qinval indexes are only up to 19 bits wide
    
    There's no need for 64-bit accesses to these registers (outside of
    initial setup and dumping).
    
    Also remove some stray blanks.
