[Xen-ia64-devel][PATCH]Patch to transform double mapping to single mapping on VTI domain

Dan,

This patch is intended to transform double mapping to single mapping on
vti domain, after this change I think it is possible to merge ivt.S
file.

This patch is based on changeset 6467: b2312c14f144.

This patch will not impact xen0, actually I have tested it, xen0 can
boot, but because control panel can't be built due to some patches not
yet checking in, I don't test xenU.

BTW, VTIdomain0 can boot after applying this patch.
