It was just because of the patch order in my local repository.
It seems to be likely that the per-vcpu vhpt patch will be accepted
before the tlb-trackig patch. So I reordered them now.

The most of ifdef can be removed.
They were there for maintenance to mark modifications because
I expected that it would take a while to accept.
I attached the cleaned-up patch which isn't depend on the tlb tracking patch.
