[Xen-ia64-devel] PATCH: use same RID for rr0 and rr4 in metaphysical mode

Hi,

as slightly discussed before, this patch makes change to use the same RID
in metaphysical mode.  As always, I did a small cleanup and added comments.

I am also working in full support (in VTi) of dt=0 and it=1.  That's the
reason why metaphysical_rr4 was recycled to metahysical_rid_d (and 
SW_V2P_DT/SW_V2P_D replace SW_V2P).
