To build a dom0 blob into xen (on PPC) simply add
  DOM0_IMAGE=<path to image>
to your make line.
This _should_ work.. please post if it does not.

When it fails it will tell you where you need to put your initrd :)

There is also a way to do it with a cpio archive but not sure how to  
do that.
Where to get an initrd is an exercise for the reader :)
