Could you able to make fs on this LV BD in dom0. If then you can try to make the file system on this dom0 then attact to  domU and mount in domU.
If u are unsuccessfull then could be something wrong in LV itself.

How I said, hda4 is in the list of /proc/partitions but mkfs.reiserfs
reports a fail at the moment of format it, and fdisk don't show hda4.
