I believe you could add

alias scsi_hostadapter xenblk

to /etc/modprobe.conf (or use scsi_hostadapter1, if there's a
scsi_hostadapter entry already), and then you could run mkinitrd without
--preload.

You also need to change the driver for eth0 after PV Dom-U runs.

alias eth0 xennet
