In my /boot/grub/menu.lst
title Xen 3.3 / XenLinux 2.6
      kernel /boot/xen-3.3.gz console=vga
       module /boot/vmlinuz-2.6.18.8-xen root=/dev/hda1 ro console=tty0
       module /boot/initrd-2.6.18.8-xen.img

and for create my img :

./mkinitrd kernel_version=2.6.18.8-xen --add-module=ehci-hcd --add-module=ohci-hcd --add-module=uhci-hcd --add-module=ide-generic --add-module=ide-disk

Are you sure that your host controller needs legacy ide rather than sata/ahci?
Do you have a working/bootable kernel without Xen support?
