Do you have xenconsoled running? 

I've seen xenconsoled crashing sometimes, causing this kind of
behaviour. If it's dead, restart it.

Also sometimes I've seen xenconsoled getting stuck somehow, ie. it's
there, but it doesn't do anything. Killing + restarting fixes that case.

Worth trying.
