Regarding root=/dev/cciss/c0p01 does it match "df -h" reported path to "/" .

No it shows as: /dev/mapper/xen4-root 

but fstab shows:
# /dev/mapper/xen2-root
UUID=9da5323b-e47c-4367-bed0-2d2b53ba5a45 /   ext3  relatime,errors=remount-ro 0   1

Shouldn't it be /dev/cciss/c0d0p1 ? 

It is c0d0p1.  It was a typo in my email but correct in grub.  

Does the initrd have lvm support? with mkinitrd it's "-f lvm2". 

I'll check.  Just need to figure out how.
