Did you try a PV install of Centos 5.4 using a Centos repo or iso? The anaconda 
installer will install kernel-xen and the correct grub entries will be entered 
(sans hypervisor- xen.gz).

Using pv_ops kernel of the day is unnecessary and might lead to all kinds of 
trouble.
