Ignore the attachment. FWIW it's a config that works fine for dom0 but hangs at boot for hvm domUs.

Can someone post some updated "gold standard" configs that are known to work for dom0, domU, (pv and hvm), and bare-metal?

This would greatly simplify getting new users up to speed if there was a set of known good configs for all (most ?) cases.
