At this point I suggest you find out what's different between the
computers that worked correctly and those that don't. Perhaps it's
some additional setting on their individual switch port. Or some
settings in native vlan. Or speed/duplex settings. Looking at both
switch console log and dom0's console log when the problem occured may
also provide some additional info (like some ports suddenly detected
as offline, etc.).

Also Centos 5.5 has been available for some time, so you might want to
update (at least the kernel-xen package), and see if the problem is
solved.
