I create the bridges in the Dom0 at boot. Put the following in your 
/etc/network/interfaces

auto br0
iface br0 inet static
        address 192.168.42.10
        netmask 255.255.255.0
        network 192.168.42.0
        broadcast 192.168.42.255
        gateway 192.168.42.1
        bridge_ports eth0
        bridge_stp off
        bridge_maxwait 5

The edit the DomU config accordingly:

vif         = [ 'mac=00:16:1E:1E:1E:1E, bridge=br0' ]

And in /etc/xen/xend-config.sxp:

#(network-script network-bridge)
(network-script /bin/true)
