This happened to me with servers having a lot of memory (100Gb or more)
This was corrected with dom0_mem=512M among other parameters in the multiboot grub.cfg file

If you are on debian, the best solution is to add
GRUB_CMDLINE_XEN="dom0_mem=512M dom0_max_vcpus=1 dom0_vcpus_pin" to the file /etc/default/grub, then update-grub and reboot
