I had to know, so I went digging for it -- see the 'losetup' commands in
/etc/xen/scripts/block-file -- this setup script is called by the
block() function in /lib/python/xen/xend/Blkctl.py, which is in turn
called by attach() in xend/server/blkif.py, etc.
