Probably you are running ntpd in domain0? You can try adding some code to the end of set_rtc_mmss() in arch/i386/kernel/time-xen.c (just before the return statement):

    struct timespec *ts;
    do_gettimeofday(&tv);
    ts.ts_sec = tv.tv_sec;
    ts.ts_nsec = tv.tv_usec * 1000;
    do_settimeofday(&ts);
 }
