[HVM][Builder] Make construction of MP tables a configurable option

This patch adds an option, mptable, to the hvmbuilder.  This option is
saved in the HVM information table and accessed by HVMLOADER.  HVMLOADER
uses this option to determine whether or not it needs to create the MP
tables for the guest.

Note: I've updated the user documentation to include the new mptable
option and used VMX to match the existing documentation.  The
documentation should be updated to use HVM instead of VMX when referring
to unmodified guests.
