SVM patch to modify the interrupt/event injection logic.

This patch resolves performance issues concerning AMD-V virtual
interrupt/event
injection for AMD-V.
- Remove extra VINTRs vmexits, and only setup fake interrupt if intr
pending.
- Allow both event injection and interrupt injection concurrently in
vmcb.

Applies cleanly to 13078.
Tested on 32bit, 32bit pae, and 64bit hv on changeset 12895.
Tested SUSE10, SLES9.3, Windows 2003EE, Windows XPSP2 HVM AMD-V guests.
