[Xen-devel] [PATCH][xen-3.3-testing.hg] Fix MSI-x remapping

This patch backports MSI-x remapping fix (c/s 18631 in xen-unstable.hg)
to xen-3.3-testing.hg. It doesn't change anything on the original patch.

The original description and changeset:

MSI-x may have multiple vectors, however in current interrupt remapping
code, one device only has one entry in interrupt remapping table.

This patch adds 'remap_index' in msi_desc structure to track its index
in interrupt remapping table.
