I am trying to commission some new hosts.  These patches fix problems
I encountered.  There are fixes to:

 * Better support UEFI x86 hosts.
 * Improve the commissioning instructions in README.dev.
 * Handle the new FreeBSD anointments system in make-*-flight in
   production (Executive) mode.
