Shouldn't that be .../initrd.img-2.6.16.13-xen [or similar]?

You may need to add the MPT SCSI device to your configuration with:
cd <xenroot>/linux-2.6.16.13-xen 
make menuconfig
[Add SCSI controller as needed]
cd ..
make dist
make install
