Same issue on an older system this morning... mkinitrd -v -f initrd-2.6.16.29-xen_3.0.3.0.img 2.6.16.29-xen_3.0.3.0 --with=ide-disk --with=sd_mod --with=ide-generic --with=ext3 --with=piix --with=sym53c8xx ide-generic is what did it for me...
