You should post your full boot logs, from both the Xen hypervisor (xm dmesg),
and from dom0 Linux kernel (dmesg).

Also enable logging for Xen in grub.conf, something like this:

kernel       /xen-4.0.gz dom0_mem=1024M loglvl=all guest_loglvl=all
