Not necessarily. http://xenbits.xen.org/linux-2.6.18-xen.hg 
has pvusb drivers for 2.6.18 kernel.

You could compile those with the el5 kernel-xen. Shouldn't be that difficult.

Xen supports qemu USB passthru to Xen HVM guests without any additional drivers.
It's pretty much the same as what KVM has.
