We just went through a similar transition from PV to HVM and this was the approach I took. I created a small block device with an MBR and a single partition for /boot and then added it as xvda. I wasn't able to find a way to tell xen to tell qemu to use something other than xvda to boot from, otherwise we probably would have used a different drive letter for even simpler transitions.
