Wouldn't a switch() be more appropriate in mov_to_cr(), and moving
declaration and initialization of the added vlapic variables into just
the case 8 scope (or perhaps eliminate the variable altogether, and
just directly use VLAPIC(v))?

This patch adds CR8 virtualization. It's the initial patch for booting
HVM x64 Windows guest, and just let every CR8 access issues vmexit.

And later we will do acceleration to it.
