The API is in xen/include/public/xen.h, in particular HYPERVISOR_mmu_update,
HYPERVISOR_mmuext_op and HYPERVISOR_update_va_mapping.

For examples of how to use it, grep your favourite Xen-aware kernel's
sources, or have a look at extras/mini-os/arch/x86/mm.c
