I applied up to here (wrt #4 and flush_tlb_domain(d) I figured that
would be a follow up patch).

#6 was acked but since the other amd change in #7 wasn't I thought I
would wait and take these all in one go.

>     - #10-#11: Add new device tree helpers

I applied these.

The rest I think had comments and I couldn't see anything else which
could go in independently.
